OSHA has increased the maximum allowable penalty amounts for violations in federally-mandated states such as Pennsylvania, West Virginia and Ohio. The new amounts are a result of a cost-of-living adjustment enacted under the Federal Civil Penalties Inflation Adjustment Act Improvements Act of 2015. The Act was intended to increase the effectiveness of civil monetary penalties and enhance penalties as a deterrent.
The most recent penalty is based on a cost-of-living adjustment multiplier, resulting in the following increases from 2019.

As a reminder all employers must provide employees, former employees and their representatives access to the Log of Work-Related Injuries and Illnesses (a.k.a., OSHA Form 300).
In addition to providing access when asked for the OSHA Form 300, employers must post the Summary only (OSHA Form 300A)--not the Log--by February 1 of the year following the year covered by the form and keep it posted until April 30 of that year. So by February 1, 2020, employers must post the 2019 OSHA Form 300A.
